First birthday cake fun? Only in America?

So as my little one approaches his first birthday I keep asking myself if I am really going to give him his own separate cake and let him dig in and make a mess. I know this is basically tradition in American culture, but I wonder if other countries (healthier countries) do this as well. I just can’t imagine letting my child eat a whole lot of cake before he is even established on sold foods, and what the icing will do to his sensitive tummy. I don’t want to be a party pooper and I know that the cake thing makes for great birthday pictures but would I be too overbearing if I didn’t do this for my son? Did anyone NOT do it? Thanks!
